Nichtalkoholische Fettleberhepatitis (nichtalkoholische Steatohepatitis, NASH)

In den westlichen Industrienationen stellt die nichtalkoholische Fettleberhepatitis (NASH) die am weitesten verbreitete Lebererkrankung dar. Die nichtalkoholische Fettleberhepatitis betrifft ca. 20–40 Prozent der Bevölkerung [9]. Ein Großteil der Erkrankungsfälle wird zwischen dem 40. und 60. Lebensjahr diagnostiziert. Nichtalkoholische Lebererkrankungen stellen wahrscheinlich die Hauptursache von Lebererkrankungen bei Kindern und Jugendlichen in westlichen Industrienationen dar [20]. Trotz der häufigen Assoziation mit erhöhtem Körpergewicht, Typ II Diabetes oder Hyperlipidämie ist die klinische Diagnose einer nichtalkoholischen Fettleberhepatitis schwierig. Allgemeine Müdigkeit, allgemeines Unwohlsein und geringer Druckschmerz im rechten oberen abdominellen Quadranten recht häufige, aber sehr unspezifische Symptome. Die meisten der NASH-Patienten weisen keine klinische Symptomatik auf [4].

Risikofaktoren für eine nichtalkoholische Fettleberhepatitis (NASH)

Die Hauptrisikofaktoren für die nichtalkoholische Fettleberhepatitis sind Fettsucht (Stammfettsucht), Typ II Diabetes, Dyslipoproteinämie, sowie das metabolische Syndrom. Die NASH ist sehr häufig assoziiert mit Übergewicht, Typ II Diabetes und Hyperlipidämie: 69–100 % der klinischen Studien sehen eine Assoziation zwischen der nichtalkoholischen Fettleberhepatitis und Übergewicht [12,21]. Ein Typ II Diabetes tritt bei 34–75 % der Fälle auf und eine Hyperlipidämie, die häufig mit Übergewicht und Typ II Diabetes assoziiert ist, wurde in 20–80 % der Patienten mit NASH beschrieben [12].

Darüber hinaus gibt es eine Reihe weiterer Faktoren, die häufig mit einer nichtalkoholischen Fettleberhepatitis assoziiert sind: komplette parenterale Ernährung, Hungern, schneller Gewichtsverlust, Hypothyreose, Hyperlipidämie, abdominelle Operationen (z.B. ausgedehnte Dünndarmresektionen, Gastroplastien, biliodigestive Anastomosen) oder zahlreiche Medikamente (Amiodarone, Tamoxifen, Glucocorticoide, synthetische Östrogene, retrovirale Therapie, Tetracycline, Minocycline). Zudem sind verschiedene Erkrankungen, wie Morbus Wilson, Weber-Christian Erkrankung, partielle Lipodystrophie, Abetalipoproteinämie, eine seltene autosomal-rezessiv vererbte Störung des Lipidstoffwechsels, oder die jejunale Divertikulose mit bakterieller Überwucherung häufig mit einer nichtalkoholischen Fettleberhepatitis assoziiert.
